Nondiscrimination Policy

(1) Heart of Hope Asian American Hospice Care is in compliance with Title VI of the Civil Rights Act of 1964 (Title VI), with Section 504 of the Rehabilitation Act of 1973, with the Age Discrimination Act of 1975 (Age Act), and with Section 1557 of the Affordable Care Act (ACA) of 2010. We do not discriminate on the basis of race, color, religion, age. sex (an individual's sex, gender identity, sex stereotyping, pregnancy, childbirth, and related conditions), sexual orientation, disability (mental or physical), communicable disease, or national origin with regard to admission, access to treatment or employment. We will make every effort to comply with these and similar statutes.

(2) Heart of Hope Asian American Hospice Care complies with applicable federal civil rights laws and does not discriminate, exclude or treat people differently on the basis of social status, political belief, sexual preference, race, color, religion, national origin, age, sex or disability with regard to admission, access to treatment or employment.

(3) Heart of Hope Asian American Hospice Care will do our best to accommodate patient's needs to provide free aids and services for people with disabilities to communicate effectively with us. Aids and services may be provided as appropriate, such as qualified sign language interpreters; written information in other formats (large print, audio, accessible electronic formats, other formats); and free language services to people whose primary language is not English, such as qualified interpreters and information written in other languages. If you believe that Heart of Hope Asian American Hospice Care has failed to provide these services or discriminated in any other way, you may file a grievance in person or by mail, fax or email by using the contact information listed below. Grievances must be submitted to Heart of Hope Asian American Hospice Care within 30 days of the date you become aware of the possible discriminatory action, and must state the problem and the solution sought. We will issue a written decision on the grievance based on a preponderance of evidence no later than 30 days after its filing, including a notice of your right to pursue further administrative or legal action. You may also file an appeal on our decision in writing to Heart of Hope Asian American Hospice Care within 15 days. The Administrator/CEO/Board of Directors will issue a written response within 30 days after its filing.

The availability and use of this grievance procedure does not prevent you from pursuing other legal or administrative remedies.
